State medical boards often require third parties to complete these verification forms and then return the documents. - Malpractice Documentation – State medical boards have an obligation to check on malpractice actions. It is possible to have one’s name attached to a medical malpractice claim without ever realizing it. - Reporting Information – A number of agencies collect and compile information about providers in allied health professions.
